I did LITERALLY every quest in THE ENTIRE GAME on this playthrough , I set foot onto the rooftop for the final battle and STILL no achievement
This acheivment is certified BROKEN , I'm on Xbox 360 btw

Did you get captured during the events at the Howe estate? I think that you also have to play through every origin but I could be mistaken on that part.

I made sure that I did literally everything there was to do literally nothing was left in my quest log and no quests could be found anymore before I went in for the final battle

Not sure. Did you complete just about every side quest? I think there's a few little locations that are parts of sidequests which done show up until you get the quest and such. Off the top of my head I can think of the Crows missions and to a lesser degree some of the 'romance' locations.

I know sme profies don;t update properly but achievements seem to be almost automatic, i also see that you hav the achievement fr finishing all the endings so I assume that yours are updating.
That being said, I do not see the achievement fr 75% of side quests done. A lot of side quests have thier own little areas.

Unless theres some secret organization of sidequests that don't REALLY exist then not only did I do "just about" every side quest I did LITERALLY all of them

A lot of the 'secret' stuff like gax'kang, and the summoning fonts also have their own little areas involved. After 3 playthroughs i thought i ha seen everything...then today found another random encounter that happened because of something I did earlier in the game. I doubt I'll get everything on this playthough, but i am waiting fr an official patch to restart my solo nightmare rogue and i inded to o very slowly with that.

Wrong, I got it without doing the Crow's final mission.
I got the achievement on 360 first playthrough, after playing every origin mind you.

Only thing is some things such as the human noble origin place counts as 5% of the world and being a city elf I could never go there , if that is why then that is ****ED UP!

He man, try this link it has specifics on every area you need to enter to get the achievement. Apparently all of the areas have a codex entry so you should be able to check to see what (if any) you missed. Apparently origins have nothing to do with it, which is good.

*points at self* I made a character for every origin, then kept the human noble and went through the game. I missed a few side quests, such as the final crow mission. But when I entered the final boss battle, I received the achievement. The reason the game is not registered, is because my brother bought it, and I'm playing his copy.
edit: Looking at the guide, I can tell you I never touched K or D's hideouts and I got it.

I went to every listed place in that link you posted and in fact more places than that even
I even have a super rare Axe called the Axameter , because thats how ridiculously thorough I was on this save I even got the rarest random encounter in the game for craps sake

I got the achievement once I hit the roof top at end game. I had a Mage origin if that helps..
There must be something you missed. I would think that it might be in Orzammar. Lots of little areas in that place.

There is one location that is only available through the quest crime wave and you can easily miss it if you pick the wrong dialog the first time you talk to slim.

Yup, got the achievement on my first playthrough with Dwarf Noble origin. I was very thorough though.

I got it on my first run through as a human mage. This is on the PC, but it rules out the noble origin. Also, didn't manage to get the D or K missions finished so those don't count either. I'm wondering if it has anything to do with the random encounters such as the meteor rock easter egg. Also, did you use Shale, because there are some areas that are shale specific that wouldn't be sure about. Although I didn't do many of the other characters specific quests and I'm sure those have unique areas.
