I have 6 characters created with the downloaded character creator (not one a human noble). Played through as my City Elf Rogue without starting any of the others. I got the acheivement when I hit the roof of the fort to face the last fight (hope that's not spoilery.)
I did not work for the Crows.
Don't know what to tell you other than I do think that many people who are adamant that they didn't miss anything are probably wrong - even if they used a strategy guide or walkthrough.

This is probably key. Some things are not obvious at all at all.

You know if you save the dogs life in ostagar you get a random encounter and the dogs there?......... I killed the dog on that save
I wonder if THAT counts as an area?

There is a quest that opens up an area that is a qunari camp where you have to slay them. This was the hardest location for me to find, so maybe that was it.

Although I do like the OP's view on things. 1 person is having an issue, it MUST be broken (with swearing). The achievement is for stepping foot in every place in the game, so you have to do that. Wish I could help you more, honestly I do.

It's just a thought, but did you visit the chantry at Orzamar, and also enter all of the spirit doors in the fade? I'm not entirely sure about the fade, but it's just possible that the destination of those doors could be classed as locations needed for the traveller achievement.

I posted this somewhere else.. but I didn't receive the achievement until I loaded the very last auto save that allows you to go back to camp after the end of the game. Of course some people are having trouble getting that auto save also..
For me it was 96% explored. game completion score was only around 60 something I think.

What I DIDN'T do this time around were the sidequests for the mages, crows and interested parties. This character was a warrior, so I didn't get the rogue sidequests. I had already finished all but one of the Chantry board quests. I also didn't do the Keep DLC or any of the companion sidequests. Where people trip up may be in the cities; looks like you need to go into every single possible shack and shanty, whether you feel like your character is intruding or not.
