I died as a Krogan Soldier while using the Striker rifle as it was ramping up and was revived almost immediately afterwards, this left me in a bugged state in which I could not move, shoot, use powers, switch to heavy weapons, or even look around as I was perpetually bugged and my ammo just drained constantly.  I was unable to move around even by using the Krogan heavy melee which just caused me to run in place.  My view and character were not reeset even after bleeding out and I just continued to stare at my bugged out corpse until the wave was cleared.  After the wave was cleared I just stood back up still in the same glitched state and was unable to contribute to my team at all except as a poor tank for the entire match, a very frustrating experience.

[Janus Prospero]

I once died while going up a ladder. My corpse got stuck on the ladder, and when I got revived I remained stuck there. Super annoying.
