Nightmare is of course anything but easy going; pausing and micromanaging (spellcasters in particular) is all but essential, but then wasn't this how the old Bioware/Black Isle games were played? I was clamouring for a return to the "good old days" and I think DA:O on Nightmare comes very close to that.
FWIW my ideal set-up is one tank (Shale or Alistair), two DPS (backstabbing rogue a must, plus a warrior) and one caster (for aoe crowd control, disabling bosses and healing); or else one DPS and two casters. Very WOW-esque terms but I was going for as generic a description as I could.

I also thought that I might as well make it as challenging as possible since first playthrough, since the Dragon Age world was so new and I thought that I might as well make it in tune with my character by being opened up to a new world and face things I had never faced before and struggle for it and being overwhelmed was also one of my expectations for that and it was met and I dont think I would have enjoyed it as much on an easier difficulty setting.
And you're right, because Easy for instance doesn't have Friendly Fire so you can pretty much use Area Of Effect spells all you want as opposed to Nightmare where you have to be quite careful as to where you fire at.

There were some boss battles on Hard that I barely scraped through though. Those will be the real challenge.

I think, however, that when I used a respecialization potion at level 8 to make my character the tank and Alistair the DPS (we were both warriors, so I figured it wouldn't affect the gameplay), I realized how much respecs affect difficulty - being able to put all the attribute points in relevant ones and change the layout of skills like that does seem a little bit like cheating.

Respeccing the NPCs probably does impact difficulty as the game was designed towards a limited level of variability...the designers knew that a certain level of party power was constant and then could factor in some 'fudge' for the variability of PC choices.
With NPC respecs, that is all out the window.

Same here! I had a lot off trouble in Hard, had to reload a lot some of the battles ended up with 20 injuries total and replaying the same char NM from start, it's been a lot easier, zero injuries so far and many of those battles done in the first attempt now.
Granted, knowledge of the game, tactics, party potential and a improved build for the same char helped but I'll try a Hard run again with a new char... seems to be more challenge!

Nightmare difficulty is stupid in my opinion. Instead of enjoying the game you're wasting your time worrying about healing potions all the time.

Not necessarily. Nightmare can be slower paced than easier settings but potion spamming is not required. Besides, there are enough AI exploits that you can get through most fights without spamming.

In any case, nightmare is fun because you can wipe from numerous things. From that lone mage doing an untimely mass paralysis or the huge number of archers scattershooting you at the same time, trying to figure out how to be victorious is half the fun. For people who don't like figuring out these kind of things, I understand where they're coming from, but for me its the main reason why I like DAO.
And there's a learning curve, but its not so steep so you'll get the hang of it. I personally paused like a mad man for the first few hours, but later on I only really pause at the beginning of battle to issue orders, or when aiming cone spells since I wiped a few times when I accidentally CoC'ed my tank or worse my healer lol.
Seriously, if the game gave you more tactics slots (or you're given the combat tactics skills for free), and if your controlled character does NOT suspend the use of tactics, you can probably breeze through nightmare just through tactics alone barring a few manually placed AoEs here or there.
