You're right, it doesn't take place in the game.
This face was made during the early stages of the Gibbed's SaveEditor. The user morphed Shepard's head to make him constantly smile like that and took a picture to prove the headmorhp function worked.
Sorry to say this, but you wasted your time trying to find it.
