When a ME1 save is imported into ME2 and a user decides to "mess around" in custom appearance, but ultimately decides to stick with their imported face, a "False" face code is generated. If that game save (with imported ME1 save and face along with it) is played to completion and ready for importation into ME3, the game will "read" what the face code is. However, since it is reading the "False" face code, the code generated will not match what was played with from ME1 into ME2.
Here are examples:
Mass Effect 1 - Jane Shepard:

Mass Effect 2 - Jane Shepard (Notice the face code present):

Mass Effect 3 - Jane Shepard (face generated from importing from ME1 > ME2 > ME3):

The face code generated is similar to Default FemShep #3 (in ME3).
There is an external work around to fix this by using Gibbed to strip the face code away from the ME2 game save, but for those that do not know how or even fear to use any outside program to modify a game file or such may not want to "risk" it.
