Yeah, don't know if anyone has brought this bug up yet - but Kasumi died in my ME2 playthrough, and she still shows up in ME3 for me.

Same thing happened to be but with Balak. I was rather surprised that he showed up on one of my playthroughs despite the fact that I killed him in ME1.

Same thing happened with me and Thane. He was alive when I intentionally got him killed(sort of a Black Widow story with my Shepard. Did it to Kaiden too.). Imagine my surprise to see a dead lover on the Citadel.

again glitchy proof that things you did in ME2 didn't really carry over to ME3.

In my case I never got to meet Yeoman Chambers in the the Citadel, even though right before starting ME3 , I replayed the last portion of ME2 and saw her survive. Only Miranda did not survive in my case.
Is that possible that former Yeoman Chambers only appears for first one or two visits to the citadel? Does she ever leave the location?

The only way Chambers can leave the Citadel is if she is killed by Cerberus during the coup. Shepard needs to give her some advice on how to stay alive.

When you "killed" him, did you shot him on the spot, or leave him to bleed out? Because he survives if you do the latter.

Killed him. As in, I put a bullet in him to make sure he died. I was affraid if i left him to bleed out he'd manage to survive so decided not to take any risks...ironic isn't it?
