I have had this happen to me and several friends, when about to die and do a biotic charge. After dying while in the charge my character becomes glitched and most of the time floats above or below the map

seriously this happens to me every single time i play a vanguard it makes the vanguard a broken clss.

This happened to me yesterday. Slugged it out through the rest of the match, was miserable.

I'm glad this has already been brought up. I just hope they are working on a fix soon. I love my Vanguard Shep, but it sucks not being able to play it in multiplayer.
