I've read and have also seen a vid about Kelly telling you her new name being Felicia if you tell her to change her identity in your first visit. However for some reason I didn't get the cutscene where she tells you about her new name during my playthrough. When I went to visit her the second time she was already confessing about spying on me. Is there some sort of trigger for the Felicia scene?

I am sure the only chance you can tell her to change her name is your first converation with her. Check this video out, and note at time frame 1:14, where the user fails to select the right option to save Kelly life.
