I like to take things down with a single shot. I always pick headshot and damage upgrades when possible. Run with Armax gauntlets and HK chestplate and legs, plus the headshot visor.
I stick with the Mantis until I get the Black Widow. The Javelin has better damage, but the warm up time before each shot is a turn off. Warp Ammo gives a boost against every defense except Shields, so I generally use the sub machine gun if the enemy's shield is too strong (Atlas, basically).
On insanity, a cloaked headshot from the BW takes a bit over 50% of a Harvester's life, and 35% or so from a Brute. The Banshees are still a pain. Everyone else loses one defense bar per shot. Which is why I take the BW, the semi auto three shots let me deal with Marauders and Assault troopers quickly.

Solid advice, but I would note that from what I've read and experienced myself, you can't get headshot bonuses for brutes and banshees. Consequently, if you are rolling with the BW or another high-damage, low-ROF sniper, I don't see much of a point in speccing for extra headshot damage, since most things besides the big bad dudes (or dudettes in the case of Banshees; gotta be sensitive with the gendered language, you know) will die with a single headshot regardless, and almost all the rest don't have "heads."
Agree with you on the Javelin though. I don't get the love for that gun. How are you supposed to be pinpoint accurate when it doesn't fire as soon as you pull the trigger? Plus, the damn thing somehow manages to weigh even more than the BW. I tried it for a few missions, and those were the only missions where I really wanted to smash my controller.

Proximity mines with the radius upgrade can hit enemies behind cover, when they recoil shoot em a couple times with the scorpion, if they are still alive cast a proximity mine at em to cause a cryo explosion.
Worked extremely well actually, scorpion makes phantoms completely trivial, just shoot at their feet. The only problem i had was with Dr. Eva, you cant kill her with a scorpion, so i had a smg as a backup for mars.

I'm pretty sure they still get headshot bonus damage. But they have such a ridiculous amount of HP that you don't feel like you're making progress anyway. At least with brutes, headshots hurt them while body shots seem to tickle them. Throwing on the cloak helps though, I'll have to test it again. It would explain why brutes cover their heads too, to make it harder to get the bonus damage.
I have found that some enemies have a poorly defined head area though. Notably the Harvesters and rachni soldiers. Hitting the eyes seems to ensure you get the bonus damage, but it's easy to hit the neck instead and deal normal damage.

P.S. As for Javelin - its exels at 2 things - killing cloaked Phantoms and shooting people through cover, and, due to "highlight" its much better in this things then any sniper rifle, Widow included. The tradeoff is poor accuracy on moving targets. BW is much more universal choice, I'd in fact preferred Javelin (since hitting moving targets with sniper rifle isnt my favorite anyway), if it wasnt single-shot.
