2-part: There is a SP glitch/exploit where the player is moving the Normandy through a system and hears the "encountered something" beep (like when getting close enough to a planet to orbit it) except the Normandy is in what looks like empty space. It typically means the player found a destroyed fuel station without having to scan for its location. If the player double-clicks with the left mouse button after hearing this sound, the Normandy will acquire the fuel without having to scan first.
Addition (exists post-1.03 patch):
At the conclusion of Citadel: Barla Von when Shepard returns to him after acquiring the Shadow Broker Wet Squad, he mentions that he's giving Shepard a finder's fee but Shepard receives no credits but receives only reputation during the final conversation and then receives only experience after the conversation ends.
Addition (still exists after the 1.03 patch):
When activating Sprint out-of-combat and something momentarily blocks Shepard's movement, Shepard will start sprinting at the correct speed once the obstruction is gone, but the animation will be a slow-motion sprint animation (akin to the 6-million-dollar man). For example: If the player is moving Shepard forward out-of-combat and activates sprint at a door that is still opening and Shepard catches the last part of the door opening, Shepard will sprint with a slow-motion animation.
I edited my previous posts with notes whether the issue continues to exist after the 1.03 patch. Red for apparently fixed. Green for still present. Nothing for those that I have not verified.

Joker's embarrassing intercom discussion with Mordin in the medical bay fires every time Shepard enters the medical bay after the conversation where Mordin sings again until Mordin leaves the Normandy. All other conversations that are triggered like that fire only once. (Not that hearing it multiple times isn't still amusing, though.)
Addition (exists after 1.03 patch)
Suen is supposedly tidally-locked with one side always facing the star. The planet rotates and has cloud-patterns indicative of planets that rotate despite this description.

It's possible that the Keepers salvaged those pieces from the citadel battle in ME1. You are told the Keepers cleaned away a lot of debris, maybe they recycled the debris for this control room.
When there is damage the keepers use the meterials at hand to repair things.
The one thing that did bother me thoguh was the bug I've constantly had when approaching Joaker on the bridge, I get stuck to the floor and cant move

(The number 780 is visible from the Presidium Commons and Huerta Memorial Hospital and the arrangement of shops is the same.)
#56. Still exists after 1.03. So fishes can fly after all... some pointed out a glitch where fishes from the fishtank miraculously get out and are "swiming" in the air, in Shep's cabin.
Addition: Still exists after 1.03. The scanning grid around Irune during scanning is much larger than the planet itself.
Addition: Still exists after 1.03. The keeper on Citadel level D-24 Normandy docking area can push Shepard instead of stopping. This is not consistent. Sometimes, the keeper stops as it shoulds. Other times, it can push Shepard long distances.
Addition: Exists after 1.03. During the mission Citadel: Volus Ambassador, when Shepard is leaving the office, Shepard can collide and pass-through NPCs walkiing around ,starting in the hallway and out in the common area.
Addition: Exists after 1.03 During the mission Citadel: Volus Ambassador with Zaeed alive and loyal, when Shepard reaches the wrong door (which is the first door Shepard tries no matter which one tries first as expected -- this part is not a glitch), gunfire sounds begin immediately even though the dialog telling Zaeed about Din's involvement with Shepard has not started. The gunfire sounds continue until Shepard activates the other door. It seems that the gunfire sounds should activate for a short period before Shepard reaches the second door.
Addition: Exists after 1.03. After talking with Vega while he's getting a tattoo if Shepard leaves level E-24 and returns without leaving the citadel altogether, Vega will not be anywhere visible on E-24, but Vega's disembodied voice will bark the card-game remarks near the counter where the kid is waiting for her parents. After leaving the Citadel, Vega appears where he should for the card games. (The map updates Vega's location immediately after his conversation even before Shepard leaves E-24.)
Addition: Still exists after 1.03. When talking with Zaeed for the first time on level E-24 on the citadel, he twists his head too far and yells "What're you lookin' at?" to someone. Then, a disembodied Batarian voice responds in fear. If this is the first time talking to Zaeed after the Citadel: Volus Ambassador mission, you immediately get the war asset, but you have to talk to Zaeed a second time to figure out why. It seems to me that Zaeed is supposed to bark the first line automatically when Shepard approaches and then give the second line and the war assets when Shepard talks to him.

Addition: Exists after 1.03. When Shepard tells Traynor to send someone up to the cabin, her voice sounds like it is in the room instead of over the intercom like when Shepard asks Traynor up to the cabin.

28. Still present after 1.03. There's a salarian doctor in Huerta Memorial who ask for some kind of biotic enhancement. When you deliver the item to him there's a heavily glitched animation which shows the doc. The camera angle seems out of place and sensless. The mission ends by gaining 90XP: I don't know wether it's a glitch or not, but you don’t get any war asset from him.
31. Still present after 1.03. On Thuchanka when you get out of the truck during the cutscene where you see the Shroud for the first time you can't see the reaper – it's missing. After the cutscene you can see it clearly.
33. Still present after 1.03. At the end of the Coup there's a camera angle glitch: after Udina and (optionally the VS) is killed everybody leaves the scene via the door: at the ending of the scene there are two pretty strange and senseless camera angles showing one of your squadmates leaving and showing an empty place with a bar.
35. Still present after 1.03. Thane's death: when Thane died his eyelids and eyes are still in motion and he’s still breathing. Hmm…
39. Still present after 1.03. Another camera angle glitch during Jacobs mission is when you take Garrus with you and Jacob says hello to him. The camera angle is quite wrong. It shows you instead of Garrus. And when Garrus answers the camera does not show Garrus but an empty wall instead.
44. Still present after 1.03. During the beginning of the last Ranoch mission (with the Reaper and the climax of the whole quarian-geth war) Shepard hands a stone to Tali. Well the stone disappears so Shep hands a piece of nothing instead to Tali.
54. Still present after 1.03. The Minos Wasteland keeps flashing throughout half the game while there's no quest available in that system. The planet Aequitas is flashed out by the green name-tag on it, but when I move there, nothing happens... you cant scan it or do anything else with it (other than read the info about the planet).
77. Still present after 1.03. For the final, slo-motion targeting of the Rannoch Destroyer, the Reaper only faces the center of Shepard's walking area and not toward Shepard if Shepard's standing off to the side.
78. Still present after 1.03. When Shepard "sprints", his squad speeds up as well, but they do not have a "sprinting" animation variation like Shepard, and they look like they're just running normally but fast-forward.

For me there are only very few bugs in the game (single player) but most of those I've found, force me to either reload a save game (Shepard get's stuck behind Joker on Normandy bridge) or stop me from finishing a quest (Hanar diplomats bug).
The other bugs I've found in single player are more of a cosmetic nature (characters turn their heads 180 degrees. Missing animations in "Aralakh/Rachni" mission) but are also very annoying when you reach those parts of the game.
I know that Bioware has concentrated most of their resources towards the Extended cut, but it would be nice, if they could look into the single player and try to fix the most annoying/bigger bugs.
Please Bioware, tyr to fix the single player bugs!

1)Occasionally I get invincible enemies, particularly geth hunters, which cannot be killed but can still shoot at me, preventing finishing the wave/mission.
2)The bug with charge where you fall through the world and then float to the top is still here, why is the default to float off into the sky?
3)Some shots hit the scenery when shooting from cover
4)When shooting from behind cover right next to another person all you can see is their back despite them standing next to you.
5)With the Krysae I sometimes lose a shot and only fire 2, and the ammo counter sometimes displays -1 ammo.
6)Hotkey 6 to restore shields does not function reliably still.
7)MOST ANNOYINGLY as an infiltrator stealth randomly breaks for no reason, usually immediately after being activated but with no shots or powers being used.
8)Sometimes shield regen gets stuck and takes a loooong time to kick in.
9)When picking up ammo it jumps to the default ammo for the gun, and then again to what it should be with the expanded clip with a delay between.
10)Sometimes the ammo boxes don't work until the next wave starts.

Your multiplayer War Assets sometimes disappear until you exit to the main menu and reload. I believe it is caused by loading the autosave after dying. Is not a big issue, but if you have a high amount of War Assets in promoted characters and then die during the last mission, it could affect what endings you can choose from.

- If she's alive at the end of ME2, Morinth is always a cameo as a named banshee during the first phase of the Hammer section of the ending.
- Jack must be rescued from the Grissom Academy or else she gets turned into a cameo as a named Nemesis during the assault on Cronos station. (You can also hear the Cerberus folks doing something to her on Cronos before the nemesis with her name and voice appears.)
- Zaeed is only during the Volus Ambassador mission, and appears in the refugee camp only after
- Kasumi is only during the Hanar Diplomat mission.
- Thane is in the hospital only before the Cerberus coup and after his email becomes available. If you do not talk to him there, he's gone once the Cerberus coup starts. If you do talk to him, you see him a couple of times more. I think one might need to make Shepard talk to the virmire survivor during the first visit to the Citadel as he specifically mentions the virmire survivor during his initial conversation.
Jordam Bau (to continue Hanar Diplomat mission) appears in the hallway between Bailey's office and the Spectre office after you read his email.

specifically, after 7:22, shep glitches. 7:40 through 7:50, shep looking
in a weird direction, shep glitching, 8:01, looking in wierd direction,
8:09 through 8:12, shep glitching/walking weird

& Shep conversation in her room, when selecting "The information
drone" dialogue it causes animation problems. also when in liaras room,
the scene when she sits on your lap and kisses you, i think when she
gets up and goes back to her terminal she glitches and flips around. ill

the dialogue between mordin in the medical bay and joker over the
intercom, when eve is present, and they are talking about sex with edi.
this repeats, i think if you complete a mission and enter the medical
bay, or if you come back? i cant remember
dynamic shadows on peoples faces is buggy sometimes. its like the shadow
is not smooth and its blocky. turning dynamic shadows off will result
in black faces sometimes. an example is when you first encounter ashley
in the beginning of the game, turn off dynamic shadows and she will be
in blackface.
cannot even play single player if my internet goes down. it complains
about not being able to authorize dlc when i hit Resume. the dlc is
authorized though, ive played through the game numerous times now and
its fine when i have internet connectivity
during the tali missions, when she is stationed in the war room, she has
animation/placement glitches. i have priority rannoch left, and i just
went to talk to her, when you select the goodbye dialogue option she
starts typing like shes facing the terminal.. but she isnt. shes now
standing here staring at her wrist, im assuming her omnitool is supposed
to be lit but its not

And during Priority: Tuchanka, while traveling through the city of the ancients, I take down my first Brute and another one appear and squadmates repeat the same dialogue.
EDIT: Oh, forgot about some of the game dialogue being cut off in mid sentence sometime, like in Citadel: Hanar diplomat and such.

In multiplayer as a salarian infiltrator with the N7 valiant I got an extra side effect of the "blood splatters at full health" glitch. I had an infinite amount of shields but nemeses could get past the shields to take down my health anyway. I could also not pick up additional ammo, not from the ammo boexes and not from the ammo equipment pack bound to 8.
