I have the collectors edition of the game with the metal case and the case is way to tight, ever since day one it has been a struggle to get the game out of the case without bending the disc. No matter how hard i push down it just will not release.
Today i took a look at the game and the area around the center hole is broken, the top layer is broken off and im worried it will weaken the game as well and off balance the disc causing ring burn. Ive had the game on my HD since day one, but i have no idea where the missing piece went so im guessing it is inside my brand new 360 as well.
Who can i contact about replacing the disc? for the time being i have the game in a different case to stop further damage but im afraid to even put the disc into the system so i cant play SP right now.

i did contact EA cs and i keep getting the run around over there. I email them then they send me back a link to the warranty page which says i need some sort of authorization from a customer support rep.
