Is it explicitely described somewhere in the codex or another valid source where you appear after travelling through a Mass Relay? In some cutscenes in the games you appear next to the "sister" Relay and in others you may appear on the other side of the system from its relay. Is it just a case-by-case basis depending on the types of relays/path taken or is it user error? Anyone know?

In mE2 it was stated that while travelling through a relay (also noted in ME1) That Drift of anywhere from a couple hundred to several thousand Kilometers from the relay is normal.
Remember the first scenes in ME1? Joker noted Drift at just under 1500k which he compared to jumping across the galaxy and hitting a needles head.
So most drift will occur, I assume, around the sister relay itself.
