My guess would be the Manswell Expedition world in the Alpha Centauri system. We know it can support human life, as it has a 'lost' colony. It's also the only world I'm aware of mention that's within easy FTL distance of earth, less than half a day for the Normandy, I believe.

It would be about 8 hours, which doesn't fit with the timeline of the ending at all. There was no way Shepard was unconscious for 8 hours, because the ground forces are only just starting to fall back when Shepard wakes up.
Basically, there is no way the Normandy was in FTL at the end, since there are no planets close enough. But it also doesn't make sense for them to be fleeing through the relay. One, the relays are supposed to be virtually instant, which it clearly wasn't at the end. Two, what was the Normandy running from? The impending Relays explosions? Great, let's escape the exploding relays by running through a relay! \\sarcasm
So the Normandy couldn't be in FTL, and it also doesn't make sense for it to be in the relay network...so there is no way it actually happened. Someone clearly didn't put enough thought into this.
I stand by my assertion that the only possible solution for this scene is to kill it with fire.
