...when you are playing human infiltrator, being the last one standing to finish the wave, only to die on the last mob - marauder shields. And then have people laugh at you over the mic.

Also feel stupid on my paper armor Asari Adept getting repeatedly revived and killed when the group is caught in a crossfire and there is nowhere to cover.

And, who hasn't gotten killed accidently running over a stray grenade you did not see.

I just did that with my shiny new kroguard.  I'll just stick to my space magic classes from now on

Getting downed 3 inches away from the extraction area with 3 seconds left on the clock.
Also on Ghost, team mate goes down at the far end near spawning area.I'm nearby as my KV thinking I could shrug off most of the damage to get to him. I go down the ladder where I did not see a Phantom waiting for me....instakills me and other player uses medigel....

When youre hunting a phantom and mutter under your breathe "come around that corner b**ch so I can shoot you in the face" just in time for your roommate to turn the corner into the living room then runs out the front door screaming "help he's gonna shoot me!"...

At least the cops thought it was hilarious. After getting tackled, handcuffed, and the house being turned upside down looking a gun.

...when you go for an unknown/unknown/gold quick match for a fun challenge, and 9 times out of 10 you end up in a lobby doing white/geth/gold. Sooooooo boring. Best of all, you leave the lobby and queue again... only to end up in the same lobby. Five times in a row.

...when there is only you + 1 player wave 5 hack because others did leave in wave 3. You use all your med-gel other too, it was little hard to get hack area. Hack ready friend go’s down you try help you die. Other bleeds dry, no tapping to keep you alive. Some reason game don’t end! On last second someone did join your game! One person against army -style. Then some teamwork and we did get full extraction

+1 This is normally when playing QE on Hydra. The spacebar will drag me into cover when I want to run, and then not work on objectives and revives. Other maps are ok. Anyone else had this?

Yes Ive had the no revive bug a few times...it really does make you look like a sadist.

When I get revived by a teammate, just to get instakilled by the banshee which appeared on top of you all of a sudden. Or Biotic Charging with the Battlemaster into a Banshee (I know I know, but it's just like driving a sports car, you can't help driving 100km/h above the speed limit...), to get instakilled the second later.

Or all these stupid moments from me running around the level trying to get to downed teammates, only to get blown up by the grenade which happened to lay in my path (and I didn't see), or jumping over cover to hide behind to get my shield/barrier recharged just to get blown up by said grenade...
Or those times I happen to be the last one alive on a wave, only to screw it up so badly.
Or when I run into a mob with my cloaked Geth Infiltrator to revive a teammate, just to die myself during the revive (before completion) because my cloak wears off earlier than expected.

With my above mentioned Krogan Vanguard those moments weren't really a problem, just BC into them (all staggering) headbutt (to stagger one), heavy melee, biotic charge again (to fully recharge barriers) repeat cycle till all of m are downed and you didn't lose one bar of health. But when that process goes wrong, THEN you have a problem.

I'm curious though, did you go after your roommate, or couldn't you resist trying to complete the match
