Like Ulrik mentioned, Duncan was seriously wounded when the Ogre hit him and the effort that went into killing the Ogre just over did him. So I can understand him leaving his weapons to stagger to Cailan.

Also, no one knows what happened to Duncan at Ostagar. In the game your PC and Alistair assume that he died, but Wynne survived the battle and it is possible that Duncan also survived but we just don't know.

If you did Wynne's personal quest u would know that she did use some kind of cheat code.

or you know, the screen turned WHITE because he just got hit in the face with that greataxe, light at the end of the tunnel sort of deal.  Wishful thinking will not make Duncan alive.
