I had everyones loyalty in the ending of ME 2, I did all paragon options, I sent Legion into the tubes, Miranda as the squad leaders, Samara as shield, and Mordin as escort, and BAM somehow TALI DIES IN THE END?! I just did an insane playthrough as an adept and this happens, anyone help me understand? I guess I will have to take tali and legion in the end, JUST IN CASE THE GAME DECIDES TO F ME IN THE A AGAIN.

Did you have enough of the right people to "hold the line", grunt, zaeed and garrus if i remember correctly. I took garrus and grunt to the end boss and miranda died for me, as my love interest from 2.

Yep, depends a lot on who's in the HTL team. mmm buddah23, who did you take as your squadmates to fight the final boss? If you took any of Zaeed, Grunt or Garrus that's probably why Tali died. You might think you want the extra firepower from a couple of guys who are good at fighting and can take a lot of punishment but you're actually better off leaving them in the HTL team and taking physically weaker techy types like Mordin, Tali or Kasumi with you.

What they said, Grunt gives them a pretty big boost I think, I took garrus and thane to the reaper boss

This would result in a character death in the Normandy when approaching the collector base, not at the end of the suicide mission.

Not quite sure how this happened, but then again I lost Jack's loyalty after Miri's loyalty mission and she survived the suicide mission. probably because I left Grunt, Garrus and Zaeed to hold the line. I read somewhere that Grunt can essentially hold the line by himself though, even if he's not loyal.

I took Grunt and Samara for my final and didn't lose anyone.

There is a thread with a detailed account of the maths. the short version is
check the characters you have left to hold the line
Grunt, Garrus, Zaeed are tough and so are+2 points each
Tali, Kasumi, Jack and Mordin are fragile and so -1 point each
also subtract a point for each character who isn't "loyal" (including tough and fragile characters)
If the result is negative - someone dies

i took mordin and i think it was jacob to the final boss and i didnt lose anyone
