It tends to happen if you are wearing the stealth mask but choose to have helmets off in cutscenes. Check your helmet and change it, then see if that fixes it.

It takes a specific combo of eye height and width to get them to look this good. But for my Bro, nothing's impossible.

I wish I had more screenshots of my shep having the issue, but I deleted them all because my inital thought was "Herp Derp screenies, delete." But it happened alot with my Man Shep, even in romance scenes (real romance killer).
