It's different every time I play a different class. I choose a team that best combines with my class. But I kinda felt that Garrus totally had advantage over Williams and I don't think I will use her if I have Garrus available :)

ME1: Ashley (because she is a soldier like me and I don't need another one with the same abilities)
ME2: Zaeed (I was afraid of him)
ME3: James (I don't know... I got used to my old team memders including EDI)

And of course Tali in all three games. There were missions in ME2 and ME3 where I HAD to take her with me, but... But nothing beyond that. I use her very rare

1 - Kaidan. Basically, never. Boring SoB.
2 - Jacob / Mordin / Thane / Kasumi - Boring / Wimpy / Boring / Meh.
3 - EDI / James. Tech? Psh. / Combat? I *am* combat, sucka.

Well, I play a soldier almost every play through of each game.my first 4 play throughs of ME1 I used each character quite a bit to allow the needed percentage of quests played for the character achievements.

as for least played in the others...
ME2 - Zaeed and Jacob.

ME3 - James and Ashley/Kaiden.

I usually stuck with Garus and Liara or EDI and Liara in my ME3 play throughs. One of the best compliment of powers with a soldier. That's just me though.

1 - Liara and to an extent, Tali.
2 - Tali, Samara, Kasumi, Miranda, Mordin and later on Jacob.
3 - EDI, Tali because she shows up so late and later on Liara.

I don't have anything against Tali, I just haven't found her all that useful for my Infiltrator across the series. And of course since I'm an Ashley fan, Ash tends to have a near permanent spot on my team in 1 and 3.

ME 1: Liara (I was always creeped out by her constantly wanting to bone me despite telling her I'm not interested)

ME2: Miranda (too much of a **** and uninteresting), Jacob (boring), Samara (boring), Thane (boring).... that's pretty much it.

ME3: James. Like most people I'd much rather spend time with characters I've known for a long time. Then there's also Liara (again) and Tali (if only because Tali shows up much too late in the game).

EDIT: forgot to mention Javik.

My selection for my squad has more to do with the powers I want to use in combat than anything else. And I like versatility, so biotics is my go-to suite of powers. I also like tech powers that aren't specific to synthetics. So the squad members that get the least play in my games are those who don't have biotics or versatile tech, which usually means the Soldier types. Ashley and Garrus didn't really get a lot of squad time in ME1, but truth be told neither did Wrex and Tali. I just found Liara and Kaidan to be too useful. Kaidan especially, he had biotics and tech, and so he came on pretty much every mission.

In ME2, naturally, Miranda filled the Kaidan role, especially early in the game. But by the end of the game I find myself using Samara and Thane as my squad in most situations, unless I know I'm fighting synthetics, then it's Legion and Tali. I do take Mordin with me a fair amount too, just because I like the character so much, and because Incinerate in ME2 is a beast.

In ME3, in my games where Kaidan is the VS, he gets to go along when he's around, but Liara is my go-to gal. And since I don't care for Javik and I roll my eyes every time I see EDI, James gets a surprising amount of exercise in the field. Garrus and Tali are alright, but I've just never bonded with Garrus on the field, he's a "conversations only" character for me, and Tali is only with me if it's a mission against synthetics.

ME1: Wrex/Tali (didn't dislike them, at all, just preferred other characters most of the time)

ME2
: Jacob and Thane. (among the great squad in ME2, these 2 may aswell never have existed)

ME3
: EDI. (She's alright I guess, but Liara+Garrus when ever possible <3

Yeah, people keep saying that. I guess I'm just such an awesome player, I never needed the assist.*

You definitely didn't play a soldier. A soldier's squad line up would be Shep, Tali, Liara damn near by default.

If Shep is soldier/infiltrator/vangard
ME1: Ashley (already am a soldier, don't need another)
ME2: Jack (gets my vote for most useless squadmate of all time)
ME3: James/Ashley (same reasons for as stated)

If Shep is Adept
ME1: Liara (same reason I don't use Ash if playing soldier)
ME2: Jack (always useless)
ME3: Liara

If Shep is Engineer
ME1: Tali (Shep as engineer is damn near the only way to make her useless)
ME2: Jack (always useless)
ME3: Tali

ME3. I really didn't use James since I took the soldier role.

If you read the insanity strategy guide, you'll learn that jacob is awesome when paired with another biotic, as you can use his pull to set up biotic explosions. He's actually thoroughly awesome when paired with Miranda and her squad bonus powers. I actually had to discipline myself to not use them so much in combination to give other squadmates a chance.
