I don't like Tali. As a squadmate or a character.

I have to laugh at the people who didn't use Tali in ME1. She was a one woman Geth wreckin' crew [with Dat Colossus X armor]

ME 2: Tali (they nerfed her pretty bad)
ME 3: Ashley (she just treated me SOOO poorly, that I didn't want to spend time with her)

ME2: Tali, Jack, Samara, Jacob even though I wanted to use him more (Used Garrus, Miranda, Thane and Zaeed the most)
ME3: Tali....... (Used Garrus, Javik and Vega the most)

Being an Engine all my playthroughs I really had no need for her and honestly she just gets on my nerves.

ME2: Zaeed (The only squad mate in all 3 games I really couldn't stand)
ME3: Javik (Didn't hate him, just didn't love him enough to take him with me often)

There wasn't any squadmate I didn't try to rotate fairly regularly, but I did tend to defer to Tali and Liara. Playing as an adept I nixed Liara and used Ash, Garrus, and even Wrex more. As an infiltrator I might nix Tali. I certainly will if I play as an engineer.

Kinda seems to me that if you never try to use them, you simply don't know how useful they are.  People that say, "I used him/her once, then never again," haven't had enough exposure to them to effectively judge their combat effectiveness.

Unless you're saying that you neglected them because you dislike their character, then it's just your loss, IMO.

That depends on the mission (enemies, layout...) and my class, usually I don't take squadmates with same/similar class. Except Garrus . Plus I tend to balance bio vs. tech on most missions.

Least used
ME1

• Kaidan (doesn't see any use after the beginning, maaaybe on Noveria - ME1 sentinel is the true "jack of all trades, master of none")
• Ashley (close second - heavy firepower covered by either soldier Shep, Wrex or Garrus)

ME2
Okay, ME2 squad is too big to use all of them with any regularity, but the ones I take only very rarely are:

• Jack (Samara's abilities are more useful to my playstyle, plus she can use ARs. Warp Ammo is really good, but its my go-to bonus power, sooo...)
• Grunt (I don't have much use for a tank, the only mission I use him apart from his LM is Haestrom)
• Mordin (funny guy, good abilities, but waaay too fragile.)
• Kasumi (same as Mordin)
• Legion (if only by the virtue of being introduced too late)

ME3

• EDI (There's nothing she does that either Tali, Garrus or Kaidan can't do better)
• Ashley (If I let her aboard at all - again, Garrus suits my needs better, and when I need a pure combat squaddie, I use James)
• Tali (Murderous against synthetics, mediocre at best against anything else. Actually slightly better than in ME2, but ME2 had quite a lot of synthetic enemies, so she was more useful there)

Most used
ME1
Liara + either Tali or Garrus on most missions, Wrex is a must on Therum and really shines on Noveria.

ME2
I tend to change it a lot, but usually my squad composition is Garrus+Tali against geth and Eclipse and Garrus+Samara/Jacob/Miranda against anything else. One combo I almost always use is Samara/Thane in the first two parts of the suicide mission. Shame both are post-Horizon squadmates, they'd be really useful there.

ME3
Liara + Garrus. I used at least one of them in every mission.

Me1 and 3 - kaiden. he's friggin useless i thought. otherwise liara in me1 and EDI in me3

me2 was jacob.

There's hardly any reason to mix it up beyond what is required by the mission mandates.

In my two main ME2 play throughs, I used Garrus and Jack in one and Grunt and Jack in the other.   The others showed up in their forced missions.

In ME3, I used Garrus and EDI in one and Ashley and James in the other.

ME1: Kaidan, something I regret, he is actually really useful.
ME2: Tali, most....useless....squadmate.....ever! She cannot fight worth crap! Only thing she can fight is Geth, and she even sucks fighting them too! (She greatly improved in ME3 though, I will tell you that. Energy Drain my love!)
ME3: EDI, I like her she is a good character, but I do not see her as a squadmate. Never used her on my second playthrough. Bring back blue ball EDI!

ME2 - Tali, Jack and Jacob(Tali was incredibly useless as she died often even when I put her away from the enemies on the battlefield)
ME3 - James and Liara (Meh, dont think their abilities are that good, stasis is cool but I can do without it)
