ME3 - Javik (didn't care much for his character, but I took him on the missions that I figured he'd have a significant-ish effect on. Extra dialogue was interesting.)

I very rarely choose squadmates based on combat skills; I tend to use squadmates I think will have something interesting to say or whom I personally like the most.

ME1 - Well, I got all the companion achievements, so I don't know.
ME2 - Only used Jack, Thane, Legion and Mordin for their Loyalty quests.
ME3 - 1 playthrough only, but never used Javik, ditched Vega and Kaidan asap, only used EDI on her mandatory mission.

God, I wanted to make this thread just a hour ago.
In ME1 Kaidan never ever left my ship. Tali and Liara got one plot missin each
In ME2 least used were probably Tali, Samara (not enough time to use everyone dammit)

Same here. Tali is like a litle sistter to my Shep, taking her to the battlefield just seemed wrongRogueMumei wrote...

ME1 - Kaidan. He just kinda fell into the "jack of all trades, master of none". If I wanted tech, I brought Tali and/or Garrus. If I wanted biotics, it was Liara and/or Wrex.
ME2- Jacob, Jack, Samara (at least before her loyalty mission). Their biotics were just not very useful on insanity because of enemy protections. Once Samara's Reave was unlocked, she became more useful, but that tended to be rather late in the game.
ME3 - Vega, EDI. I just never really needed a tank, and I didn't care for Vega's skillset, so he barely saw any use. EDI's skillset was useful on paper, but the lack of cooldown bonuses meant I was better off taking Garrus for tech.

ME1: Tali and Liara (I didn't hate them, but I considered them more of support staff than soldiers).
ME2: Tali (for the same reason mentioned above).
ME3: EDI's robot body (the only true Mass Effect squadmate I genuinely despise)

Least used....
ME1: Got all character awards but after that, Wrex was least used.
ME2: Easiest to say I just used Garrus and Tali as soon as I could.
ME3: VS, Liara, Meathead, EDI.
