For me it was the last mission, War assets do not appear at all, the mission was such a horrible way to end the triology, "go to the thanix missiles and hold of waves of enemies". I was so frustrated and mad when I played trough it.
Most disappointing thing in the game apart from the ending.

apart from the ending, I would say the dialog choice. seemed to be more linear dialog compared to 1&2

yeah, i was expecting the suicide mission on steroids, also the heavy amount of auto dialog bothered me

I was really disappointed when I got an email telling me Kal'Reegar died. One of three Quarians that could be called badass, and all he got was an email.

If you discount the endings the biggest failure of the game to me is the almost TOTAL LACK OF DIVERGENCE.
The game is really linear and developer choices to minimize the amount of content one group would see vs. another group is IMHO what would kill the replayability.

The beginning - why the hell are the reapers attacking Earth and not the citadel even though Bioware spend an entire game explaining just how awful it would be if the reapers got control of the citadel...
Nothing the reapers do in ME 3 makes any sense and to top it all of they are in the last minutes reduced to nothing but tools, Man imagine if that Happened to Sauron...wait I'm going of a tangent here.

Kai Leng as the final boss. Scratch that. Kai Leng in general was a disappointment. How many times are you going to run away and hide in a single fight? And he can't even do much himself. He almost was beaten by a dying man, needed a gunship for cover and magic impervious shield regenerators while reinforcements drop in. And somehow that all wasn't "too video gamey", but a final boss fight with a character we knew was.
There's a reason why everyone but Tom Cruise died at the end of The Last Samurai. And why knights in shinny armor got replaced by lines of riflemen. Swords won't beat guns. Ask any Jedi post Ep.3 just how effective guns are. Gimme a pirate over a ninja any day.

The story, auto dialogue, rushed quality, I think they wrote themselves into a corner with the machine gods as the main antagonist

That MP can affect SP in any way - not just required for the "best" ending, also effectively disabling the worse ones in the long run.

Tali's face not being revealed on Rannoch at the perfect moment. The level of stupidity required to think that a poorly photoshopped picture would be better is so high that it would affect humanities average IQ by about 50 points.

In ME1/2, you could be wandering around somewhere, chatting to people, then have to draw weapons and start shooting your way out of trouble - then go back to a hub-style area for more dialogue, all in the same mission.
In ME3, there's a simply rule of thumb:
- Casual outfit/no guns = talking/shopping.
- Armour/guns = shooting gallery.
Combine that with the option to remove all choices from the game (as if they've not already been pared-down enough...), and it really smacks of EA/BioWare appealing to the lowest common denominator - i.e., the 'everything must compete on the same level as CoD' mentality.

Besides the ending?...... "Priority Earth"..... The mission that was supposed to be the climax of the series.... ends up being on of the worst missions of the whole series.

That a lot of really good characters from both the first and second game died offscreen. Along with the ending and MP affecting SP
