Please tell me that by doing multiplayer on Silver this weekend that I still qualify for the 1,000,000 phantoms pack - I know I won't get the squad survive pack, but they didn't say anything prior to the weekend that the phantom kills had to be on Gold. Here's hoping.
One wierd thing kept happening to me - although I set the options on 'Quick match' to Cerberus, occasionally we ended up fighting Reapers. What's that all about?
Well everyone who participated on any difficulty level (even bronze) this weekend should get the pack for the 1,000,000 phantom kills (if we've reach it, which I am sure we did).
The change of enemies usually happens when the host/leader of the game switched to another enemy after you searched and joined. Happened to me several times.
Actually, you don't even have to participate; the Victory Pack goes to everyone who has a copy of the game.

I'm
pretty sure (but not entirely) that that counts, since the squad goal says successful extraction, not playing a full match, and the whole squad gets the pack even if only one of them gets extracted. So, I don't think how many waves you were in actually matters, as long as you guys got a "Mission Successful" message at the end. Hope that helps
