I had to contact EA support.
I got the packs, but the commendation pack isn't showing up under user entitlements.
Is it supposed to show up?

Yes, no commendation pack, servers still behaves erratically, but at least my online pass (total idiocy for PC users) is valid again.

As an Xbox gamer I contacted support through http://help.ea.com, got an e-mail reply yesterday saying my account was being update to give me the pack, still no pack today.

Did not get my commendation pack either. Got the victory pack, but I did a couple of gold matches during Operation Exorcist and still no commendation pack

It's in the help of the origin client.
