They need a name to put on the file. I guess Quarians might just number their orphan babies though.

Theres a FF about them adopting one whos mother died during the orphans birth, so the baby didnt have a name.

I guess that works if they happen to be around at the time a mother dies giving birth. Though in most cases there will be a father or other immediate family to take the baby. This woud be especially true for Quarians who are very family centered. History also shows Human society tends to become much more family oriented during/after harsh times such as war. With all that advanced medical tech death related to birthing must also be a vary rare event by itself.

This adds up to the odds of such an even being very low. But there are going to be a lot of 
orphans whoes families got killed by the Reapers that Shepard and Tali could give a loving home.
