I recently just finished my playthrough on insanity, yet I did not receive the achievement for completing all the single player missions on that difficulty. I checked the journal multiple times; all of the missions are done. 

Is there a way to resolve this without going back and starting over?

Your signature is your answer. Unless you want to beat all the multiplayer maps on Gold, but that is WAY harder.

Are you sure you didn't miss a single mission? Even the side missions like the N7 missions count. You may only need to play through to the mission that you missed, rather than the entire game. You just need to look at a Walkthrough FAQ and figure out which one you did miss.

If its truly a bug, that really sucks. The worst part of it is that even if you do play through the whole game once more there's no guarantee that it won't bug again.

On the plus side, Insanity is super easy and goes through relatively fast if you do a New Game +. So there is always that.

My insanity run was a new game plus; I spent almost thirty hours on it, doing everything.

Doing that again does not sound appealing, nor does doing all of the maps on gold.

When you look at that achievement in game, does the meter read that 27/27 missions were done? I originally thought I had done everything on my insanity run too, but when I checked that counter I found I actually missed a couple of missions.

If you decide to do another NG+ run, only the N7 missions and story missions count towards the achievement. That should save you 10 hours in fetch quests and planet scanning.

It does say 26/27, but I can distinctly remember doing all of the missions, N7 and misc. quests such as Grissom Academy. 

I did put off the Rachni mission with Grunt until after getting Tali, though. Perhaps that could have something to do with it.

Hmm...if you still did it, that should have counted, that's weird.

Good news is you can (if you feel like it at all) go through the game on the Narrative difficulty, and just turn it to insanity before doing that mission again.

So it's possible to keep it on narrative but change it to insanity before N7 and misc. missions? I'll try that, I suppose.

Absolutely, it's the only factor that made me want to continue pursuing the achievement after I found out I didn't actually do all the missions on my insanity run. Otherwise....

This is sucks.

Happens with me too.
I unlocked the Tour of Duty achievement (Complete all 6 N7 missions) on my insanity playthough, but the counter says 3/6, but I double-checked the journal and all 6 missions is completed.
The Unwavering achievement counter says 24/27. Probable the missing missions is the 3 N7 missions.

Anyone know how fix this?
