I assume you are referring to the questions in part5a/5b. This is first asking your overall opinion of it currently (fine as is, more information, change the situation), then its asking you to rate how important/unimportant that particular issue is to you. If you are ok with it as it currently exists in the game, you would mark fine as is, then unimportant/very unimportant. If you want more stuff with the squad members you would mark change the situation (narrative/logic) and mark it as important or very important to you.
The survey tries to list the main complaints the majority of people seem to have. The choices that are available allow you to say that everything is fine as it is with the current ending(and don't change anything), all the way to, everything needs to be fixed.

No the site controls the ads but if I had anything to say about it I would definitely feature ads for WWP, it seems like a very noble organization.
