I perfectly understand the reasons for limiting the amount of questions (I do questionaires for research myself, from time to time), however, I missed a couple of things under either the "major themes" or the "other issues" section:
1) Events should be explainable within the lore of the game
-> Mass effect is relatively thorough with scientific explanations, but we do not learn a) how Synthesis is supposed to work,
2) Narrative problems with the last
"decision": Previously, major decisions were typcially backup up by
enough information to judge outcomes. We do not have that with the last
"decision".
3) (As hinted by 1b) Narrative problems within the last mission as a whole (not only the last 10 minutes), in particular:
a) Why do the reapers move the Citadel to Earth (and not to some remote location where noone finds it)
4) Logic problems with the If the hologram kid was in control of Citadel, why was sending Sovereign in ME1 necessary
I would have expected to see at least 1 and 2 in the survey.
Please note that I appreciate the effort you put into this a lot and that the survey surely isn't bad, at all - I would just extend it a bit.

I Wish SO much that actually bioware makes an acceptable ending which will make me crave for multiple playthroughs of Me3. So far only one and i cannot do another one.
In me2 i performed 7 playthroughs and i could do more given the chance that my choices would actually matter in Me3.
Bioware Fix the game please!! we are doing the best we can for feedback!!

I want however to point out that for another sequel of Mass Effect series life or death of Shepard and of his squadmates isn't important. I can think of making some party based semi-tactic RPG set in time after Reaper Wars where you can create almost any character you want to play with of any of current ME universe races (Baldurs gate, anyone? Bioware's own trademark). Setting will be based on ideal choices from previous game without the ability to import savegame.
What is, however, important issues of Mass Effect 3 (ending) must be addressed and ending adjusted to be logical, have to provide closure to Shepard story and also should be as optimistic as possible within a set storyline (ec. galatic cilivization should prevail, removing Reaper's threat, but Shepard reunion with his LI to make a perfect happyend is optional only)
