I'm trying to play the entire game on insanity. I'm on the final mission, on the final streach, I did every other mission in the game, every side mission, every N7 mission, and that one trophy where you have to beat all single player missions on Insanity or all MP maps on gold, it's still telling me I've only beaten 24 out of 27 missions and I did everything. Is there something I'm doing wrong here?

And another question: If I start my insanity playthrough, then I go to play a save in a different file that wasn't on insanity, can I still get the Insanity trophy? Because if not, then I'm playing for nothing and I'll have to go start the game all over again... And this is the first time I ever tried to beat a game on a difficulty harder than its easiest difficulty! If I have to start again, I will be pissed!

From what I understand the last mission is counted as 2 towards Unwavering. I'm wondering if the Cerberus headquarters mission doesn't count until you finish the final quest. When did you switch to Insanity? Was it at the beginning of the prologue or was it closer to the end? Because I noticed I got 2 for the prologue and Mars and I switched right away when I gained control of Shepard in the prologue.

For the Insanity trophy you have to beat the game without changing difficulty after leaving Earth.It doesn't matter how many missions you do only that every one you did to the end game was done on Insanity. Its locked to your profile (i.e. John Infiltrator) so if you have multiple saves within that profile changing it in one will make the change in your profile. Although backup and restore via the cloud works, like for easier hijacker trophy. But as far as Unwavering goes you can't change it one bit in the profile, do so and you'll miss something and have to start anothr profile (career) to start getting credit again.
See here: social.bioware.com/forum/1/topic/346/index/10859238 for what I went through getting Unwavering and what I went through for Insanity and an unconfirmed guess that as long as you do ALL Priority missions on Insanity you should get the Insanity trophy. I say that because I did, in fact, change the difficulty for the hijacker trophy in a side mission, restored and still got Insanity trophy when finishing that profile. I did though lock some Unwavering out so be mindful of this, hope this helps.

Edit: Prolouge doesn't count towards Unwavering.Read through my link, got some credit on another profile from Priority missions I HAD ALREADY gotten from the Insanity 1st playthrough..weird but meh.I got the Platinum so

I believe there's a bug with the Insanity Trophy on PS3. Not sure if it's limited to just our console or the other platforms as well though.

I wonder if you get credit for Priority Citadel 1 the moment you set foot on the Citadel after Mars then, because that was where I first checked my Unwavering progress and I didn't do anything on Insanity until I started this playthrough, haven't touched the difficulty since.

You should be on 24 of 27 before assaulting Cerberus headquarters. That counts as 1 and Earth counts as 2. I have a list of all the missions that count in my Prima Guide so PM me if you need me to look it up.

So glad it didn't bug out for me when I got it, Kai Leng's last fight and Grissom Academy were nightmares for me.

Seeing as though I beat the Insanity play through yesterday and the damn trophy glitched, I felt it was necessary to bump this.

What the hell happened!? I got the Insanity trophy, but I didn't get the Unwavering one! I did ever mission and it's still telling me that there's one more that I didn't do. This is the last trophy I need before I platinum the game (I've only platinumed one other game on my PS3), and it won't work for me. Am I missing something, or did the game just glitch up so I'll never be able to get this trophy?

The only reason my unwavering trophy is glitched is because my profile settings file is bugged and the counters don't work as was intended

(My Unwavering counter is stuck on 2/27) which is preventing me from Plat'ing the game too.

Not sure if Eden Prime (the DLC) is counted too

I did the DLC mission too and it's still stuck at 26. Do you have to be connected online for the counter to work?

Did you mess with the difficulty settings at all or have the game freeze up on you at all?  I had several trophies get stuck at one less then I needed but the trophies still popped.  Unfortunately you may need to do another playthrough on insanity in order to get it, perhaps one of the missions glitched and didn't count.  I've read about others having that issue and were able to get it with an additional playthrough.

Damn. Insanity was hell for me (even though I only died like 6 or 7 times during the course of the game) and I don't feel like doing it again. Maybe I'll try again in a couple of years...or something...
