Sometimes the Reave sound bugs out for me, and stays for the rest of the match, constantly chipping away on my sanity.

The Geth heavy melee noise is annoying when it bugs out and lasts until you melee again. I'm not sure if other people can hear it though because I've never heard it coming from someone else.
The announcer sometimes bugs out at 45 seconds till extration and doesn't say his line right, sounds like someone whispering over the mic, it always freaked me out until I found out what it was.
I've gotten used to reave, actually, I kinda like it.

The worst sound for me, by far, is the Phantom hand blaster. Every time you hear them start firing you think "Great, if 2 or 3 of the 4-8 shots fired in each burst hits me, I go from full health and shields to downed without being able to retaliate. Hoo-f***edy-raah..."

The sound of other players. Seriously, some people's head sets pick up every darn noise in the whole house. Thank the force for the ability to selectively mute players.

Just a fyi for those on the 360 having irritating-kids/mental-equivalent-with-mics-syndrome, I usually make a party and just not listen at all. Or if you are hanging on to the small glimmer of hope that at least someone on your team isn't irritating but one is, Mute 'em. 
As far as irritating noise, none of them really bother me. The Banshee's scream is less annoying and more strikes fear into me because 95% of the time I get OHKO'd -.-

Reave. You can't escape it. Try to run to the other corner of the map from the adept, the volume doesn't diminish.

Pretty much any noise a human vanguard makes because they just don't shut the hell up. Plus Raptors. They sound like an underwater fart bubble.

And a couple strange ones: the sound a Talon firing makes. Sounds like a hammer, not a gun. I can't pinpoint any good reason for it, but it seriously grinds my gears. Not 'cause it's repetitive or heralds something negative, I just hate that sound. Second, the weird almost beeping sound Claymore makes after it fires. Not quite as annoying, but it sometimes gets to me something awful. xD
Reave and flamethrower turrets I've kinda learned to tune out. They almost ends up drowned out by the above annoying sounds anyway.
Now for something completely different: some of my favorite sounds are the geth digi-speak sounds, particularly those made by the GI and GE (like when they're hit, almost like grunts of pain), and the steady thrum the Javelin makes in scope. I find the former adorable and the latter soothing.

Banshee scream doesnt bother me on some days, and really bothers me on others. Human Female Vanguard battle cries are also a contender.

I don't really understand why people don't like the Banshee scream. Yes, it's loud and high pitched, but it doesn't last for very long and it also lets you know one's on the field so you can keep en eye out of it to try and stop yourself getting insta-killed or it lets you hunt it down to finish it off before you or any of your team gets insta-killed. Reave just drones on and on and on. A couple of games with a Drell Adept or Asari Justicar using Reave and I get a headache and have to stop playing for a few minutes. Which is annoying if I've got with a team that I enjoy playing with. It's gotten to the point where I consider leaving any lobby I join if there's a character with Reave in it.
