So I was just going through my achievements, and looking at what I'm missing, what I need to do, etc (on my second play through).
I noticed that I did not have credit for the achievement Rock and a Hard Place on the 360 at all.
This is a list of my achievements so far:

You'll notice I have the other Shale achievement unlcoked (near the top of the list). This achievement should have been unlocked near the 3-5 of November.
Anyone know how to get it unlocked? Getting Shale into my party just doesn't do it.

It turns out it's a bug with the 360 achievements and a bit of brain farting on Bioware/Microsoft's side.
