It would help if you disentangle the questions about MP from the SP. I don't play MP but even if I pick the option where it says "Never" I still have to answer the other MP related questions....of course if you filter that accordingly later on it will be fine!

Nice survey. My only feedback would be that it assumes we have a reasonable amount of experience with multiplayer. There should be a "Haven't played it" or "Only played it once" option.

you should add "not applicable" options to your multiplayer questions, since one of the possible answers to "how often do you play multiplayer" is "never" (which was my answer. I ended up picking the rest of the answers more or less arbitrarily)
