Wow. This is nice. I've broken Phantom sword before, unintentionally of course, with pistol when I was aiming for her head. But I never notice Banshee blue glow or Brutes and Atlas condition. Thank you for this!

This unfortunately BS. It is stated a running phantom will not instant kill you but I have seen it happen several times.

it will if it had jst made a melee swing. Have yet to be killed by phantom w/o melee first. Note the targets of melee and instagib can be differentStardusk wrote...

Yep, the aura had just fallen and having hard-coded myself to remember these rules I hit her just as she started walking(no longer able to teleport of course), snatched me up and put me out of my misery. The Phantom thing is crap too, a Phantom can grab you whenever it wants and I've experienced Splinter Cell tier Phantoms who just snatch you, end you, and disappear around a corner before anyone sees a thing.
My Atlas correction is correct from what I've seen, it WILL NOT grab you unless it does a melee first, which makes taking down an Atlas even on Gold a telegraphed fight.

I appreciate the guide a lot, OP, but unfortunately a lot of the instakills I've dealt with are less the result of melee/vanguard classes as they are of people being cornered, turning a blind corner, or being rezzed.
Lag often renders all the rules you've indicated pretty useless, since the discrepancy between what you see and what the game records can readily result in an instakill even if it's a difference of a few 10ths of a second.
Often times I get instakilled for situations I couldn't conceivably have reacted to; for instance, I'm shooting a Banshee from around a corner; she teleports over to me, and I start to run immediately-- too late, she's pulled me in.
I'm getting up from being rezzed where my team is being assailed by a flock of Phantoms. As a Turian Sentinel, as soon as I get up I hit the nearest Phantom with a chain overload, and as soon as I do she OHK's me. No stagger, no chain, just death.
There are also times when the Biotic Charge animation directly syncs to a OHK (experienced on multiple enemies); I'm not sure that should be allowed to happen at any time, whether or not it's coded that way. That's a major screw to Vanguards and severely limits their utility and survivability on Gold.

I was just picked up and stabbed by a Banshee who did not have the glow around her. It did not kill me though, it just knocked me out.

Sadly banshees will instagib you even without the blue glow around them.

I've found that it's usually safe to Biotic Charge a Banshee when she is going through the animation of throwing her Biotic ranged attack. If you do it as soon as she begins the animation cycle, you'll end the charge when she's still in an animation cooldown and can't react. There's a narrow window of opportunity here, and if the ranged attack is already in mid-flight then it's too late. Her attack will impact you when you're invulnerable. Immediately roll/dash away once you impact, resume shooting her until she goes to throw another one.
Only tested with Humans, Drell, and Asari. Krogan can't dodge away, and their charge takes longer to perform, so I wouldn't recommend it.
Phantoms:
Batarian Soldier can tear them apart if you build for maximum melee damage and power damage. You will want Blade Armour for the melee damage boost and the melee damage reflection. Ballistic Blades should be evolved for damage and explosion. Also taking a pistol stunner or shotgun bayonet helps too. Hide behind cover until they get really close, then pop out and fire the Ballistic Blades. As long as the Phantom has not been staggered within the last half a dozen seconds or so, she will be staggered now. Immediately go for a normal melee combo, not a heavy melee (Heavy Melee charge-up can sometimes give her a chance to recover from the stagger). If the melee combo doesn't kill her, the subsequent explosion of the blades will do it.
The real danger with this approach is not the Phantom, but her Cerberus friends near by. Do not do this if there's more than a couple Assault Troopers, Centurions, Engineers, or Guardians nearby, because they will gun you down while you're exposed and fighting the Phantom. The Phantom will probably get one palm pistol shot in before you stagger her, so take that into account if your shields are not at maximum.
EDIT: There's also two bugs that can ruin this approach.
1. A bug where instead of your normal melee combo, you do a single backhand instead. This backhand attack won't kill her, leaving you vulnerable to being OHK. Sticking to cover once will reset your melee back to normal.
2. A bug where sometimes your melee combo whiffs into the air and doesn't lock on to her. This might be lag dependent.

I had an interesting thought while reading the Phantom section. Can Phantoms only use their insta-kill on non-shielded players? As OP stated, they mostly slash you with a normal sword attack first, which takes away your shields.

Had just escaped Brute OHK. Was doing my first match as a Manguard, and forgot about DAT ASS attack vector for them. Got grabbed by a dying (he had 1 sliver of Armor left) brute and while he was tossing me in the air Infiltrator managed to kill him. So, it is posible to escape his attack, but I guess it is hard because window is so short.

I get OHK by banshees a lot. Don't even have to be close. Only time it is safe is if they are doing something else. if they are standing tho, be across the map....
