I had that happen once. Insta-died. I hadn't saved in a while.

Ever since, I would make Shepard park the Mako outside the flat circular plain (always an obvious Thresher Maw trap) and have Shepard and crew run into it instead. If you make Shepard stay at the very edge of its "stay-put" range, Shepard can safely strafe its goo-missiles. The crew sometimes gets slimed to death, but they're expendable here anyway. They're good to go once the Maw is down.

(... and the massive amounts of on-foot XP is nice, too.)

I would take nearly everything in this thread, hire back the voice actors and make a gag-reels DLC. Barring that, I would try to find a way to override-mod the game to access a gag-reel somewhere in the game, even if it's just subtitles. (Both are beyond my powers, though.)
