Shepard had no role in Aria's fate, Mordin's death can be averted. Kelly's potential fate is her own fault, as is Udina's, Jenkins's and Legion's. I will give you Anderson, since you kinda shoot him.

Hmm, but from another pov
If you follow the renegate path actually you will shoot Mordin, or there is the question would Mordin be dead if you never met? Probably not. And yes you can avoid Mordin's death but than you have to kill Wrex if he is alive and if not than probably you will wipe out the Krogans.
Kelly: ok if she is a refugee it's kinda her own fault but her death is on you both time (ME2/ME3) because you have the ability to save her.
Aria: I think TIM attacked her only because you are allies and she can help you.
Jenkins: You were his commanding officer so you were reponsible for him.
Ok I admit Udina and Legion isn't your fault.
