The Citadel, an uneasy Aria T'Loak in a high security environment would have made for a very amusing biotic fight between those two old enemies. And of course, he has stories to tell.

While I haven't read the comic, I'm guessing The Patriarch didn't survive?

As for an NPC we never saw again, I wouldn't have minded seeing Eddie Lang, the C-Sec officer down in the Wards down the stairs by Doctor Michael's clinic, again. Not to mention Calyn and Xeltan. Need more elcor.
Also, much as I wanted to punch him, seeing Delan again, the jerk on Horizon who blamed the Alliance for the Collector attacks, just so we could get a chance to give HIM the Shepard punch.

Shiala. They made her so intriguing then whipped her away back to Feros.
Gianna Parasini. Great character, great comic relief. I was so pleased in ME2 when she shouted out "Oh my god! Shepard?!" on Illium, Did you like to help her or screw her over? lol
Helena Blake. She could have helped the war effort either as a merc or a social worker.
Detective Anaya. What a great dry hard ass cop. There were some classic one liners from her in Samara's tale.
Morinth... enough said.
Fist. He was possibly your first proper villain to take on in the ME universe and made an appearance in ME2.
Talitha. Awww. Who wanted to give her a hug eh?
Vido Santiago. WTF happened there? The ME3 codex refers to some generic blah about a batarian being the founder of the Blue Suns?!! Serious oversight, particularly if he lived through Zaeed's loyalty mission.
There's probably more.

I always laughed when I went past Chellick's office on the way the the Embassy Bar because the door automatically opened and he was just sitting there.
I had two stupid scenarios in my mind:
1. Chellick goes mental because the door keeps opening and shutting, sending him crazy, "Can someone PLEASE sort out this door's sensitivity, all this opening and shutting is driving me to distraction!"
2. Chellick looks up and thinks "Ooh! has someone come to visit...? No, it's just the door opening and closing again...*sigh*"

Toombs would also have been nice. Leaving him out just leaves loose ends, especially if you have a sole survivor Shepard.
