Not going to Grimson Acadamy in time. This Phantom does shout Jack trademarks, and there are dtatapads lying around with Cerberus scientists talking to her before they cut her open, and your squadmates and Shepard do comment on her you kill her.

It has to be. Otherwise, how could you trust anythign you see or hear in the game? That would mean the Catalyst was lying.
I wonder if you put an image of Casey in-game, if the nameplate will say "Harbinger". Admit it, it would expain a lot.

I would have liked to have a bit more to the Morinth encounter than a Banshee that just gets its name changed. Hell even if they got rid of the Banshee that appears if Morinth was killed in ME2 so there is a consequence for choosing Morinth. But no they have to be fair to everyone, resulting in your decisions meaning squat.

There's no justification for choosing Morinth over Samara. Whatever Morinth gets in the end is too good for her.

My first playthrough, Jack was loyal and it still happened (I delayed going to Grissom because I wanted to take Kaidan along, mistakenly thinking that's where his biotic students would be; by the time he got out of the hospital the Cerberus coup happened and Grissom was sealed off.)
As far as the nameplate's knowledge, the Jack Phantom is labelled "Jack," but EDI also confirms it is her. I don't think anyone else does the same for Morinth, since nobody knew about her. (Maybe Liara would, being the Broker, not sure.)

Really? She never had a choice in life, she was either going in a prison for the rest of her life or being executed and I know that if I was her I would have done the exact same thing, better live a short real life than live a thousand year in a prison...
