Marauder ****ing Shields had me reloading seven ****ing times on insanity. I would miss one headshot and shepard died
the only other places where I died were Grissom Academy after Jack (STUPID SPACEBAR) and that room with lotsa turrets at Cerberus base (screw you turrets)

Beginning with the highest difficulty (on insanity):
1. Grissom Academy - reloaded >100 times
2. Geth Primes (so f@#$%^.......)
2. After human Reaper in TIM's base - last run (engineers made it really frustrating)
3. Kai Leng fight in the TIM's base (tried 15 times)

Kai Leng's fight for me, until I realised you could repel his melee attack.

Second would be the 3 Geth Primes. I actually dread that bit. It seems you have to get lucky with cover and not have them crowd you out from all directions.

The toughest?  Marauder Shields, for the reason already posted: unless you're a Soldier, you are forced to play in a different manner than you've been playing for the rest of the game.  Even Soldiers lack access to Adrenaline Rush and ammo powers, although they're at least used to straight-up gunning enemies down.

The others are pretty class-reliant, like the amphitheatre fight in Grissom Academy: tough for my Adept, a cakewalk for my Engineer.  Or they're reliant upon finding the "trick" to getting through, like the Rannoch Reaper fight: I face-planted a lot until I figured out how it targets you.

The final Earth battle is kinda hairy, though.

Honestly, the only time I found myself annoyed with my inability to manage something in less than three tries was the situation after Shepherd is blasted towards the end, and is in lala land with only a pistol before arising to the Citadel for the last time. It's probably funny to most, perhaps it is because I was really tired at that point pushing towards the end, but it bogged me down for quite some time.

The Rannoch reaper gave me the most trouble until I figured out how far I needed to run and when.

Kai Leng's phantoms killed me a few times before I managed to prioritize and down them quickly.

I don't have this problem with the Turian Platoon rescue mission, but I do have this problem with the "Clear the Camp" part when you go get General Victus for pretty much the same reasons. The cover is all pretty awkward and at bad angles for me. If I go out into the open to try to get a better position, the Marauder's eat me for breakfast. Dealing with all the Brutes on top of the Marauders is just plain painful, and trying to penetrate deeper into the camp to get to the Reaper Blackstars without getting cut down by Maurader fire ... well, I've never gotten it to end well for me.

I don't have problems with Turian platoon, I think position I choose is considered to be trigger area for "retreating Shepard", since when I try to move out of it, Harvester one-shoting my Shepard. Or maybe it's just tough luck, I don't know. So I prefer to entrench (so to speak) Shepard right there and carefully take down everyone game throws in my direction. No haste, no running, just gunning them down. Crusader and Eagle helps a lot.

Regarding Victus Camp - I prefer to run on the left (farther from Victus, Brutes and Marauders), there is L-shaped cover. Indra or Valiant with IA/AP carefully take down Brutes and Marauders with little to no hussle. IEA helps tremedously. If this is + playthrough, then Widow/Javelin/Black Widow could be used, but I think they are a bit slow. Raptor with IEA, AP mod and spare ammo mod also makes wonders, but make your trigger finger tired. Image IPB
Black star no needed with Valiant/Indra. Garrus overload/James Carnage+frags (and your own, if you have some) also helps to deal with Marauders, since Brutes are melee-only and not so dangerous without firing support.

This has made me realize that I don't know anything about the level/cover layouts in ME3, having only played a Vanguard.  Cover was almost never necessary, so I didn't notice it too much.

Playing another class would be like an entirely different game in a way that it wouldn't have been had the Vanguard been remotely balanced.  At any rate, I'm glad to hear I'm not the only one that has some difficulty with that fight on Tuchanka: Turian Platoon.

I've even had this issue with the Vanguard. A charge doesn't take out the Marauder's shields, but evenif you strip the shields first and use the shotgun to deal with the rest of the Marauder's health after impact, the other Marauder's will take you out in the interim. And that doesn't even begin to tackle the three brutes you get on hardcore (I haven't started on Insanity yet). I can set up biotic explosions with reave/charge, but this one scenario still gives me fits.

Exploiting Nova gets you through the brutes really quickly, especially if your squadmates are setting up combos for you to detonate.  I typically pick up the Blackstorm and bounce from enemy to enemy while dropping Nova when it makes sense to do so.  Once the brutes are toast, I Blackstorm the Harvester...

...unless it kills me while it's landing, which usually happens.

I am on my first insanity playthrough and am finding it surprisingly easy. I am using a NG +1 Infiltrator with an ME1/2 import. The toughest battle was the courtyard of Grissom (grassy area right after saving Jack/students).

I can't think of any other areas that may give me fits but am dreading the three Geth PRIME units on Rannoch and Earth at the missile trucks.

Defending the Thanix-Missile-launcher (btw: What the heck is a Thanix-Missile?).

Banshees + Marauders including the fact that the all-in-one-wonder-button did drive me crazy.

Grissom Academy was hard in my first playthrough, in the second I realized that I could stay in the starting room with the benches for quite a while. The enemies wre staying outside and after a while I went out and finished the Atlas. Lots of engineers with the turrets was hard but doable.
Afterwards I went through the "tunnel", opened the door but went back to the "starting area". Using a sniperrifle against the enemies ion the "other side" made it quite simple.

The Thanix Missile sequence in Priority: Earth only gave me trouble when I stopped paying attention to the Reaper in the background, which eventually got me lazed. That's it. Blew through it with no trouble the second time.

Grissom Academy... the big fight in the courtyard area is only hard because your starting position is terrible. Just head up the ramp and commandeer the shield battery and you and your squad are golden. The Atlas can't even get you from there IIRC, so you can take it out at your leisure. The second part of that requires a bit more run + gun b/c you are again starting in a killzone of sorts. This whole mission just requires you to be a little smart about positioning, people.

I just beat Insanity and got my last 2 achievements. Insanity and Unwavering.

So now ME 3 joins ME 1 and 2 as 100% complete. 52 of 52 on ME 3. And, yes, I am proud.

The only problems for me were Kia Leng's last fight and especially Marauder Shields at the end. Other levels I had to keep checking that the difficulty was still on Insanity.

The posts in this thread, and others, helped me greatly on many levels.

So, Thank You.

Finished up an Insanity run yesterday, only bit that really gave me any hassle was the bit on Thessia where you have the two Harvesters pinning you down right before the temple

I don't know about the toughest but the most frustrating is any fight involving Kai Leng.

Grissom Academy was hard the first time through on Engineer. On Insanity as Vanguard, pretty easy. Only time I died was because I was testing just how far you could push your luck before the game decided to kill you, and turns out pretty far.
The game in general is just not that hard.

I had insane trouble on Eden Prime. Playing as a vanguard, I went there as soon as Eden Prime popped up and only had James and Liara. So when I saw three ****ing turrets set up, with 3 assault troopers, 2 centurions and 3 engineers (ready to repair the turrets instantly, obviously) I nearly cried. I knew what a pain turrets were since I played the multplayer demo. But nothing could prepare me for this. Seriously, wow. As soon as I managed to drain the turrets shields, the engineers immediately reparied any damage I caused, small as it was. The centurions kept throwing smoke and pushing towards me, while the assaults were laying down suppressing fire. It was awful. I still can't believe that I made it through all that :S

Now when I look back, it was actually a lot of fun! At the time though... Man I shudder at the thought. My team combo wasn't at all geared towards taking out shields or armor. No overload, no incinerate, nothing. It was a god damn pain. Then I got EDI and the game became easy mode ^^ Oh right, should've mentioned this sooner, I was playing on the hardest difficulty.

I'd say

Grissom Academy, after meeting Jack.  Guess it's because of the area design rather than the enemies, but always gives me trouble
3 Geth Primes on Rannoch
Final battle with Reaper Destroyer, Banshees and Brutes.

(Please no "It's all really easy" type posts.)

Hm I like your list. I definetly agree with the Grissom Academy, **** my life. I could barely move an inch without getting shot at from some dude I couldn't see from across the map!
Rannoch wasn't really that bad for me. There's Spitfires laying around there, just pick one up and have fun.
Final Battle however, not gonna say anything against that one. That one was tricky dicky.

1. Definitely the final Reaper destroyer battle with all those banshees and brutes.

2. Harvesters and Ravagers on Thessia before the Temple entrance.

3. Grissom Academy Atrium. Difficult, yes, but man, was it FUN!

The Rachni Tunnels in the Attican Traverse - husks were on top of me within the time it takes to say 'take cover'. On several occasions I had a companion needing first aid as soon as the fight began. I think for about the first 6 attempts the 'Critical Mission Failure' screen came up within seconds - and that was on normal setting!

Far more difficult (for me at least) than the Grissom Academy mission.

Rannoch reaper I had to tone down the difficulty for that one. That last wave of brutes and banshees on earth can be brutal but it is managable. what annoys is trying to get into cover while I take out a ravager then I overlooked some stupid husks that manage to pester me enough for the ravager to take me out while I get locked in that animation fighting with a husk.

Even on insane I never hit a difficulty choke, just an occasional stupid mistake. If you are good with squad commands and know what you are doing the whole game is cake. The last battle can be a little tricky. That took me three tries on Insanity.
