After the Citadel Coup mission, extension to Garrus by the memorial wall if Garrus was present and Thane died. Unromanced version.

Garrus: So. New name up there.
Shepard: *long sigh* Yeah. Yeah, there is.
Garrus: What happened? He actually beat me down the stairs.
Shepard: He... lost a lot of blood.
Garrus: We'll make the bastard pay. Twice for Thane.

Garrus: I'm sorry, Shepard. *long pause* Yeah, I know that's not enough. Are you going to be all right?
Shepard: It's not like I have a choice. I have to be.

Someone needs to comment that Cortez is never found very far from his shuttle, at least, if there's no romance. If there is a romance, someone should mention the surprise when they couldn't find him by the shuttle last night.
