I at first rekindled my romance with Liara at the beginning of the game when she first asks if there's still an "us." I then broke up with Miranda on the Citadel but after seeing her reaction (crying and walking off hurriedly) it made me feel crappy in real life. (I know that's cheesy but it's true. I wasn't expecting her reaction.) So, I'd really like to rekindle with Miranda again. However, I've progressed pretty far into the game since then and I don't really want to reload an earlier save. And as I understand it, telling Liara that you two are still involved doesn't necessarily lock the romance in at that point.
It actually comes later in the game after Priority: Citadel 2, when you meet Liara in the market. When she says something about wanting to spend more time together as friends, you have the choice to say you want more than friendship or that you're essentially fine with just being friends. If you choose the friendship route during that conversation, can you still rekindle the romance with Miranda when you see her later on back at the Citadel?

That only applies to the VS, Allers, Samantha and Steve.
ME2 romances like Miranda, Jack, Tali and Garrus have to be extablished in ME2 and rekindled in ME3 when they ask. If you break up, it's over.

I'm pretty sure you actually have to explicitly break up with Miranda in order to end her romance. You can say you want to be with her and then initiate/continue a romance with someone else and she doesn't find out. The only times where you can't do that are rekindling/starting romances with your current crew.
