Hey there my collectors edition disc 2 has gotten a crack, I looked around but couldn't find how to do a disc replacement anyone have a link?

This is a two disc game do I need to send in both discs or just the cracked one? Ea chat person didn't give a clear answer, is there a warranty phone number or something?
