Shepards renegade speech final mission, it was just embarassing I should have just picked the no speech option. Hacketts speech on the normandy, ditto.
Mars, the whole unshielded robot scene after the shuttle crash.

Mars - I mean, really? "We've had it for 30 years and just found these super duper plans." Yeah ok, I can understand there's a load of information in those archives but the super plans? Here are my thoughts "Oh here we go... uber plans, wooo yay /sarcasm. Should I go into more detail? Ok. Why are the plans on Mars when Vendetta the VI is on Thessia?
And the VS getting their head banged in? Shep, you have a gun. Don't just watch!
Palaven - Whoohoo, we get to go to... ah just no. A moon? Aw come on!
There's more, but I'm actually getting a bit depressed here.

The Crucible. No matter how many War assets you gather, it gets finished. No matter how much you suck through the course of the game, it just doesn't matter. Just seriously bad.
Anderson being born in London and Shepard being surprised/shocked TWICE. Seriously, it's a small flaw, you might think, but a careful, cautious writing team would not have made such a blatant mistake. Either they just really fell asleep and forgot that they already mentioned that small fact, or the writing team was not the same. I dunno which is worse.
The Earth-bias in ME3. No matter your Shepard's background, he gets railroaded into caring his ass off about Earth above all other things. I can see it with an Earthborn, but a Spacer Shep who wants to save everyone, not just humans? Seriously? I cared more about how things went on Sur'Kesh or Irune than about Earth.
London. It was rushed, no matter how you put it. Worst rushing I ever saw. I expected it to take 3-4 hours to get through it, it took little over an hour. Sorry, but that's unacceptable.
The Cerberus Coup. I liked the general setting, lots of action and pew pew, but what exactly was its purpose?

There are times where the camera is just looking at Shepard or someone just staring in a blank/generic/basic face-pose.
There are times where it seems to me that someone should really be saying/yelling/screaming something and nobody's saying anything during the action sequence.
The "extra" Mass Effect.
I enjoyed the minigames: the drunk rhino, the tissue paper tank, decrypting, bypassing, hacking, car-chasing and even a bit of the scanning. They were filler, sure. But I enjoyed them.
This time, we got the main game with the occasional turret minigame. You don't even get a reward like a temporary varren pet for the turret minigames.
(I would mention Steeeeeve [if you don't have Vega with you] and the Hammer massacre with Krogans support no matter what, but I consider those to be part of the ending.)

Same if you don't have an LI in ME3 .
In one of my FemShep runs, Liara asked Shepard to meet her on the Citadel (post Citadel Attack).
Liara says it's nice to be able spend time together as friends.
If Shepard were to trigger the love interest - saying she'd like to be more than friends (which I did)
Liara will appear all flustered and 'hadn't thought about it' etc. but does end that scene with saying 'yes' and then kisses Shepard with a fade out.
That was quite forced reponse as Shepard had given NO indication to Liara that she liked her 'that' way.
I must say that I did prefer the initial meeting on Mars - and the 'I missed you Liara' if you already had her as a LI in prior games. and later on when Liara sits on your lap - nice touch.
But then in stark contrast there was no physical emotion from Shepard when Liara is sitting on the bed with tears streaming down her face. after her ordeal on Thessia and subsequent run in with Javik.
I would have hoped for Shepard to take Liara and hold her close and just comfort her -- saying sweet nothings and what have you.
But it wasn't meant to be I guess

Yup, I love the romance but these points have always annoyed me.
Not to mention being friendzoned after being loyal to her for 2 games, telling her I'm interested and doing LotSB!! Argh!

The whole Cerberus arc as well. Rather than the great ideological conflict they seemed to be building up between Shepard and TIM, TIM just becomes Generic Evil Overlord #968 who does pointlessly idiotic things for the sake of enemy variety.

Yes, that is problematic too ... beyond the beginning (and yes, ending). And that kind of stuff comes only from laziness of game developers.

i was ok with "steeeeeeeeeeeeve" if you'd befriended him throughout the game. It was the subsequent "I'm ok"... "You sure".
and then his "anything for you" when my shep was straight just seemed weird.
as for falling on it's face, any moment where they spliced in basic character B in place of a dead squadmate. Legion dead/never activated fine we have a "geth AI' that looks EXACTLY like Legion and sounds just like him, has the same dialog (pretty much). Tali dead, no problem different Admiral will just step in and have basically the exact same dialog. Mordin dead, no problem, could have been someone else they wouldn't have gotten it wrong.
Pretty much the only one of those they got right was Wreave instead of Wrex. Some similar dialog but Wreaves approach is very very different from Wrex.

and to the guy that said this thread makes him realize what a joke the game was. I don't think it was a joke I just think threads like this point out how, while the game was incredibly fun and emotionally gripping at points, there was SOO much wasted potential.

The Crucible - Too convenient and too early. Having Hackett tell
Further evidence or the whereabouts of critical information about the Crucible would be uncovered by T'soni onboard the Normandy and then Shep would embark on several quests to find these pieces/clues. There could've been the opportunity to visit Junthor, Klencory and Armeni in order gather information, clues and evidence. Javik could've been more than a spear-chucking space douche and really had some input with these quests that never happened. The story of the Crucible should've also been the story of the Reapers.
Kai Leng - Should've been cutscenes until the boss fight. While the hand-to-hand fight with Thane was cool, nothing but really thin plot armour was keeping him alive. Seriousy, the guy was a b!tch and it's too bad my Vanguard figured how to use his omniblade at the moment where he kills Leng as opposed to ramming a biotic fist through his face.
Udina/Coup - Why is he councillor again? Remember that time I picked Anderson? Yeah, I know what you did in the novels and comics but uh, don't let those pick up the slack for the writing when it comes to what you guys at Bioware do... MAKE VIDEOGAMES.
Cerberus sleeper agents and operatives simply infiltrating and taking over C-Sec would've been more plausible but perhaps too videogamey in comparision with the all out invasion of the Citadel.
And as for the continued mistrust from the VS, maybe some more in game events/evidence that made Shepard look bad would've made this mistrust feel more justified. It was hard not to shoot the VS just out of pure frustration and annoyance at this point.

Kai Leng is so awesomely terrible, but since he was supposed to be badass, he falls very flat.
