So I was at Firebase Hydra and for some reason I started glitching and flying up randomly, which was not caused by a Biotic Charge.  Also I kept dying and reviving myself as I was falling upward.  Anyone else had this happen to them?

No, but I have seen a few vanguards get stuck under the "floor" before.

On firebase giant I ended up floating upwards until I hit the 'ceiling' of the level (way way up in the sky) whereupon I froze, unable to move, but I could quit out from the game, which I did, because no enemy could kill me but I could not even see, let alone kill any enemies, and all my team mates had bought it already.

the only real remedy for the time being is you have to host the game whenever you want to play as a vanguard
Otherwise prepare for some horrid glitches
