When importing a completed mass effect character for a second run through in Mass Effect, I was unable to switch the Combat Difficulty to Insanity until after Jenkins' conversation - do I need to quit, change the difficulty to Insanity from the main menu and start over from Anderson / Udina's conversation to be sure to get the achievement or do you think I'll be fine?

Gah. I decided I better be safe and restart. This time, I didn't notice until after getting Ashley, before discovering the beacon was moved that it auto-selected Casual as the Difficulty setting. I've switched it to Insanity ... I'd really rather not start again, so does anyone know from what point in ME you need to be playing on Insanity to get the achievement?

I believe you need to change it before landing on Eden Prime. For best results, as soon as you have control of Shepard aboard the Normandy (after talking to Joker and Kaidan), switch it to Insanity.
You may be able to switch it Insanity on Eden Prime before your first fight and it may work, but I cannot confirm.
