I don't think you need to play DA:O first, it'll just help you understand some of the references and in jokes. Gameplay-wise they are quite different.

Some of your choices in DA:O will impact DA2, but only in minor ways. DA2 does a pretty good job in giving nods to how you completed certain arcs in DA:O, but as the games are set in entirely different countries you don't see very much. None of what you do in DA:O affects the main plot of DA2.

There is no Paragon/Renegade Good/Evil system. Mostly what you pick in DA:O is your starting race and class. Each Origin gets a different starting quest. For the City Elf Origin, gender matters. Other than that, it's all down to how you finish quests. Every quest has a 'side', and being a villain, hero or pragmatist is up to how you want to play it.
