I pre-ordered my copy from Gamestop because of the pre-order bonus DLC.  The employee at the time told me that I would get the pre-order bonus when I pick up the game.  When I went to go get the game a different employee told me  the pre-order bonus DLC was on a card inside the case.  Well, fast foward to yesterday.  I finally got around to playing the game and when I looked, all I found was the Blood Armor and Shale codes.   I went back this morning after hearing that it was supposed to be a seperate card not inside the case and the manager told me that I was SOL because they ran out of them and to make sure to get to the store earlier next time as they tend to run out of these things since the publishers never include enough to cover all the pre-orders at all stores.

I was thinking about contacting Bioware about this but I no longer have the receipt so I can no longer prove that I did indeed pre-order it.  Would any of you guys have any suggestions as to what I can do, if anything?

I had to ask for mine at Gamestop too. They did have them, the employess just didn't realize they were separate apparently.
