It's a BS explanation, but the "Shepard lives" clip was nothing more than an Easter Egg for people who played multiplayer. Shepard's basically dead, at least until EC comes out with more context for it.
And high enough EMS and Shepard's Cereberus implants make him/her REALLY fire resistant. Yeah...that's honestly my best shot at trying to explain this.
The rules of this thread are hurting my brain.

Or maybe we'll get a new scene of Shepard running back to the transport beam, and only making it back to Earth with a high enough EMS.

As it stands it's physically impossible. And considering you can see a Mako behind Shepard in the rubble at the end they're definitely on Earth, which is adding a whole new layer of impossibility. Sense is not something you should be looking for with the current ending, because there isn't any.
EC seems to be changing this to Shepard surviving the same way all the other important characters on the Citadel are getting their deaths retconned, emergency automated sheltering barriers and bulkheads.
Or maybe we'll get a new scene of Shepard running back to the transport beam, and only making it back to Earth with a high enough EMS.

IT can fall a little short when it comes to the scenes of things Shepard could not personally see such as the blast, the Earth, the pulses and the Normandy's fate. Yet, all that happens before Shepard breathes so it is possible that it can be all inside Shepard's head regardless.
