IT claims that Shepard beat the Reaper Indoc and woke up still in London *where Shepard never left*.
IT can fall a little short when it comes to the scenes of things Shepard could not personally see such as the blast, the Earth, the pulses and the Normandy's fate. Yet, all that happens before Shepard breathes so it is possible that it can be all inside Shepard's head regardless.

What we don't see between the cutscenes.
Shepard reprograms the beam of white space magic and slides it down to earth before the Citadel blows up.
If you listen carefully you can hear the 'Whoohooo!'. It is very silent though.

Shepard survived a freakin fall from like ten thousands kilometers in Mass Effect 2, while grasping from air. With enough luck he can survive falling from space along with some Crucible rubble.
EDIT: I know he technically "died" in Mass Effect 2, but his body and brain were safe enough for Cerberus to rebuild him without cloning him or building a Shepard-AI. So I guess that "Shepard 2.0" here can survive such fall.

I have several questions concerning the destroy ending (even though I believe it to be the best one)
- How did Shepard survive atmospheric reentry. With no armor to speak off and the citadel in pieces shouldn't he have burned in orbit?
- When the citadel blew up, how did the onboard eezo not wipe out everything nearby?
- The relays blowing up. Death or stranded?

And if the game lets us experience everything through Shepard's eyes who eyes are we seeing through when the Citadel fires the beam? When we see the Reapers leave the Earth or are destroyed? When we watch the relays blow up on the galaxy map? When see the Normandy fleeing? I think its the entire shift of perspective at the end that makes the ending so intolerable.

Well, we only see the "front" of the Citadel in the explosion right? The Crucible docks in the "back" of the Citadel? Wouldn't it be hidden from view given the camera angle we have?

he had taken Kai Leng's plot armor and was wearing it underneath his regular armor like underoos. That is how he survives a reaper laser blast and a fall from orbit. that is also why his "burnt armor" looks nothing like the armor you have on before the laser hits you.

Well if you have a low enough EMS then Shepard dies presumingly along with every other partially synthetic person in the galaxy (most people, certainly all biotics, quarians and soldiers) and if your EMS is high enough then all partially synthetic beings live, good thing the reapers aren't partially synthetic... wait...
