I agree with pretty much everything that's been said, but I'd like to add PLEASE KEEP THE BLACK EMPORIUM MIRROR OR SOMETHING SIMILAR! (and I never shout, so you know it's important).

Based on your dialogue choices, or random? - I don't actually know, but aside from reloading to fix my Hawke I would consistently go "dangit - I HATE Leandra with those bangs!" and reload just because I didn't want to keep looking at them for 30 more hours. What a bizarre thing for the company to spend zots on - especially with all their talk about emphasizing each npc's "signature" look.
I dont think you have to worry about the Black Emporium not returning. It was already hinted that it would be back in the next game. The developers joked that it was a chain.

Agree with the edits. It saves me when I get the cheek bones wrong (and it is always the cheek bones) and in cut scenes it looks goofy from angles I didn't see in the editor.
As for the character editor before or after...meh. Doesn't much matter. I understand the approach of getting you into the game and not loading up and slapping you with a big screen of numbers. What I will say is that the ME2 method or Skyrim (shorter granted) where you really don't see yourself works better than having some not-you character running around like ME2 did. I care more about that than when the editor happens.

As for the Mirror of Transformation, there's no reason why that needs to be an in-game item. Just give us a menu option to change our appearance without trying to make the feature fit in the game world. It's a metagame feature. Just accept that and put it in a menu.
