Go with "it was all a dream" aka Indoctrination theory angle. If there's one thing that's worse that the current endings, that's it.
I don't hold out much hope for the EC, tbh. The problems with ME3 are deep-rooted and systemic, even if BW slaps some fluff on them, it's still polishing a turd.
The worst way the Extended cut can end?

Shepard wakes up in the back of a VW bus in the 1960's. He's really a dude named Carl (regardless of whether or not you played femShep) who sports a pedo moustache and bellbottoms, and LSD is a helluva drug.

Dr. Chakwas, I think he/she' waking up...
or
Commander, this is Miranda, grab you gear in the locker...
or
Not a glitch, the sedatives aren't working... SECURITY!!!
Take your pick. Still three options, artistic integrity not compromised! Depending, of course, on your EMS, the colour you've picked in the "original" ending, iridium imported from ME2 to ME3, and some other unknown factors! M.

The worst way it could end is with a question, rather than an answer.

The worst way is if the Extended Cut fills in the plotholes and explains the fate/rebuild of the galaxy... Then, with the same music as the current ending, show the newly free species developing Synthetics that eventually kill all Organics.
I would feel teabagged by artistic integrity.

You realize the entire story was just a couple of starfleet officers watching the events of Mass Effect unfold on a holodeck.

"Broccoli, you're fired. Now report to transporter room 3. I want you to beam down to the planet's surface. Mr. Crusher, set course for the nearest ion storm. Broccoli here is going to go visit a parallel universe, hopefully one that's been overrun by Borg."

Absolutely worst ending?
Synthesis is made canon and displayed as the best choice, the Crew is forever dead/lost, Mass Relays are really destroyed
you can't imagine the rage!

The worst way it can end? I'm thinking if it turns into an X-rated hentai flick starring Harbinger
Harbinger: THIS HURTS YOU!
Shepard: <CENSORED>
yeah...that'd STILL be an improvement...worst way is what they seem to be setting us up for though.
Giving us the same end result of the Starbrat dialogue but with more scenes showing the results of our choices...woo.

erm... don't think things can get much worse than they are currently.
